Canon 5D Mkiv + Canon 85 mm f/1.2L II @ f/1.2
This was shot using the Brenizer Method, with my 85 f/1.2, wide open. It is comprised of 14 images. The end result is an effective focal length of 71mm and an aperture of f/1.0. (The lens that would be needed to achieve this photo (composition, perspective, and depth-of-field) in a single shot.)
Bokeh Panorama/aka The Brenizer Method: It's the same principle as a typical panorama, except instead of shooting with a wide angle to give as much depth-of-field as possible, it's shot with a longer focal length, to give as much of a shallow DOF as possible. This can mimic (depending on final capture) the look of a large format camera. This method increases the effective sensor size of the camera, much like the larger negatives in medium and large format photography.
